Compassionate Support for Independent Living
LifeLink Behavioral Health provides services to adults across Wisconsin who are enrolled in Adult Long-Term Care (ALTC) programs. Our focus is helping individuals live safely, independently, and with dignity in their own homes or community-based settings.
What Is Adult Long-Term Care (ALTC)?
Adult Long-Term Care (ALTC) is part of Wisconsin Medicaid’s Home and Community-Based Services (HCBS) system. It supports adults who need ongoing assistance due to aging, illness, or disability while allowing them to remain active members of their communities.
ALTC includes programs such as:
- IRIS (Include, Respect, I Self-Direct)
- Family Care
- Family Care Partnership
Who We Serve
LifeLink supports:
- Older adults who need help with daily living
- Adults with physical, intellectual, or developmental disabilities
- Individuals who want to live independently at home or in community settings
We work collaboratively with participants, families, IRIS consultants, and managed care organizations
to ensure coordinated, person-centered support.
ALTC Services & Supports
Through ALTC, participants may access a range of supports to promote safety, wellness, and independence. LifeLink helps coordinate services such as:
Personal care and daily living support
- Help with bathing, dressing, grooming, and meals
- Light housekeeping and laundry
- Medication reminders and health monitoring
- Companionship and daily living support
Companionship and community participation
- Social, recreational, or therapeutic programs
- Community integration activities
- Adult day services for structured daytime support
Health and wellness services, including nursing or therapy supports
- Nursing or therapy services
- Behavioral and counseling support
- Caregiver training and education
Home modifications and assistive technology
- Home modifications (ramps, grab bars, accessible bathrooms)
- Assistive technology or adaptive equipment
- Emergency response systems for safety at home
Respite services for family caregivers
- Short-term relief for family caregivers, in-home or in an approved facility
